Changes for zSQLgate, version 1.01

Changes between versions 1.00 and 1.01 (Tue Oct 9 11:23:21 2007):

• Include sample script for System-V init, in examples/init-script.
• Add Oracle version of books example, and notes on working against an Oracle back-end.
